Application forms

Hello,

I have been doing day to day supply work for a while and am now applying for permanent positions. In application forms when it says current employment do I put down the supply agency or a school where I have worked regularly?

Sorry if this has been asked before, I did look.

I can't remember it being asked before, but is there a way you can put both down? Say something like:

Current Employment
05.05.06 - Present Day:
Really Useful Teaching Agency - working in various schools around the region, predominantely Silly Street School, Crayola Way School and Button Moon Juniors
