Default writing terms and conditions

Hi,
I have jsut registered as a self employed supply teacher and am currently trying to write terms and conditions for some jobs coming up - what do i need to include?
I have put something about not being liable to fill job if I can't make it etc and about a percentage fee if the school cancels. is there anything else that i need to put down?

Even agencies couldn't offer this so I'm not sure whether you'd have any luck!
I worked for an agency a year or so ago and the job was cancelled as I parked in the school carpark after a horrendous 45 minute journey! The school argued that it was unavoidable and so wouldn't pay anything as I'd been called before 8:20 (I managed to argue for travel expenses from the agency but they weren't happy!!)
